Description: MasterPro Installs Corp is additional insured under general liability and auto liability on a primary and non contributory basis as required by written contract. General Liability policy covers damages from appliance installation including water damages. In the event of cancellation regardless of the reason, the policies listed have been endorsed to provide 30 days notice of cancellation. Waiver of Subrogation applies on favor of MasterPro Instals Corp.

Additional Technicians/Helpers: First and last names of any technicians and helpers that will need to complete background checks.

Drug Screen: As part of the background check process, an 8-panel drug screen (Amphetamines, Barbiturates, Benzodiazepines, Cocaine, Marijuana, Opiates, Oxycodone and PCP. ) is required. The drug screen may be completed at any clinic of your choice. Once you receive your results, please send us a copy via e-mail. For it to meet compliance, there are 3 specific items that the results must include. We have listed them below:


- Must show a unique identifier with the name- like a SS#, Driver’s License number, Date of birth.

- Results must show what drugs/types were tested and that the results are negative.

- Results must indicate that a urinalysis was completed.

- Results must have a doctor’s signature (Medical Review Officer, typically).
